How to Obtain Membership
| i) Download the Enrollment application form from the website or you may submit Online application |
| ii) Submit following documents along with the dully completed Enrollment application to reach |
| Certified copy of the relevant Entry Qualification(s) |
| Certified copy of the National Identity Card (NIC) |
| Documents confirming the payment of Enrollment Fee (Paying slip & Bank Counterfoil / Copy of the Online payment Receipt) |
| Two Certified passport size Photographs |
| Note: Certification as true copy by a JP / Attorney at Law / Gramasevaka / Bank Manager / School Principal etc. |
| iii) You may post the application along with above documents/ hand-deliver to reach |
